V posledních 30 letech je endovaskulární léčba základní terapeutickou metodou léčby přímé karotidokavernózní píštěle (KKP). V současné době je metodou první volby transarteriální přístup s uzavřením přímé KKPtrombogenními spirálkami. Vzhledem k anatomii píštěle, jejímu rozsahu a kolaterálnímu oběhu jsme u naší pacientky použili k okluzi přímé KKP koronámí stentgraft. Dosáhlijsme okamžitého uzavření zkratu. Vývoj koronámích stentgraftu tvořených pouze jedním stentem zlepšuje jejich flexibilitu. Lze je lépe implantovat, a proto se může stát tento postup častější alternativou ke klasickému ošetření přímých KKP.

In the last thirty years the primary terapeutic method for dealing with direct carotid-cavernous fistula (CCF) is endovasculartreatment. Nowadays the preferred method of choice is closing the direct CCF with trombogenic coils through transarterial access. When taking into account the anatomy of our patient´s fistula, its size and colateral circulation, we used a coronary stentgraft for closing the direct CCF - with which we achieved the immediate closure of the fistula. The developement of coronanary stentgrafts made with only one stent increases theirflexibility, it makes them easierto implant and therefore this practice can become a more often used alternative to the classic method of treatment for the direct CCF.
